EXHIBIT"A" <br /> SCOPE OF SERVICES <br /> Funding for this agreement has been granted from the AOD Abuse Prevention Partnership <br /> Grant from the San Mateo County to support coordination and implementation. <br /> Specific tasks will include: <br /> Coordination <br /> • Facilitate Monthly Prevention Partnership Meetings <br /> a. Agenda development, logistical coordination, notes and next steps <br /> • Facilitate 46 Community Meetings/gatherings/events <br /> a. Provide coordination, planning, facilitation, follow-up, and logistics for <br /> community engagement events at least once a Quarter <br /> • Facilitate connection between PCRC resources, specifically but not limited to Family <br /> Engagement staff and Pacific Islander Community. <br /> • Ensure ongoing communication with contracted and non-contracted partners with <br /> monthly in person or phone conversations <br /> • Provide 1-2 meetings with each individual contracted partner identifying milestones, <br /> solidifying contract agreements and conducting year end reviews <br /> • Ensure appropriate County reporting and data collection requirements are met <br /> including but not limited to attending monthly Provider meetings, entering and <br /> maintaining Prevention Partnership monthly CaIOMS records, Quarterly Reports and <br /> Annual Reports. <br /> • Work with staff at John W. Gardner Center to identify indictors, develop and implement <br /> evaluation plan for tracking success and impact. <br /> • Document efforts of Prevention Partnership to track indicators. <br /> • Work collaboratively with RWC 2020 Executive Director in update RWC2020 website <br /> and messaging and marketing materials by December 2013. <br /> In addition, PCRC will provide Youth Development Trainings and Mentoring as part of their <br /> Empowering Youth Initiative. <br /> Youth Development and Mentorinq <br /> • Youth will be mentored and coached as emerging leaders to take an active role in <br /> creating opportunities where they can educate their peers, families and the larger <br /> community on how to reduce access to alcohol and other drugs. <br /> Consultant will invoice RWC 2020 monthly, providing documentation of actions taken during that <br /> proceeding month. <br /> REV: 09-26-13 MLG <br /> Page 10 of 10 <br /> ATTY/AGR.2013.172/AOD Subcontract with PCRC FY 13-14 <br />
