, . .. � <br /> EXHIBIT A <br /> SCOPE OF WORK, FEE AND TIME SCHEDULES <br /> Description: Demolish existing hot tub area inside the police department building and perform a <br /> design-build remodel to expand the workout facility into aforementioned area. <br /> Scope of work and fee schedute: CONTRACTOR shall furnish all labor, equipment and <br /> materials necessary for the completion of all tasks listed below. Total compensation for the <br /> successful completion of tasks below shall be$18,744.00. <br /> 1. Demolition of existing facilities <br /> a. Remove wall and door separating area between hot tub and workout area. <br /> b. Remove hot tub and all associated equipment. <br /> c. Remove tile flooring. <br /> d. Remove ceiling. <br /> e. Dispose of materials and waste. <br /> 2. Design-build Remodel <br /> a. Install new flooring to match existing workout room floor. <br /> b. Install new electrical sub panel on short wall of demolished hot tub area. <br /> c. Install a total of six(6)new electrical outlets as follows: <br /> i. Two(2)dedicated 120V outlets for a treadmill and stair climber <br /> ii. Three(3)outlets total—one located on each wall of demolished area <br /> iii. One(1)outlet on ceiling, location to be specified, for hanging television <br /> d. Install new ceiling with transition to existing ceiling <br /> e. Install custodial closet <br /> 3. The new build-out will include: <br /> a. Metal stud wall, drywall,drywall finishing matching existing,painting, concrete <br /> leveling of hot tub area <br /> b. Additional two(2)ceiling lights <br /> c. Relocation of one(1)fire strobe fixture <br /> d. Temporary plastic for shielding of construction dust <br /> e. Best effort to match to all existing finishes <br /> f. Set of as-build drawings showing new construction <br /> CONTRACTOR shall make application for payment upon completion, to the satisfaction of the <br /> CTTY,of all work listed above. <br /> Time Schedule: <br /> CONTRACTOR shall start all work within two (2) weeks of the City's notice to proceed.
