!�RIGIN�.� <br /> AGREEMENT FOR PROGRAM SUPPORT AND COORDINATION <br /> FOR THE ALCOHOL AND OTHER DRUG (AOD)ABUSE PREVENTION PARTNERSHIP <br /> GRANT <br /> FRIENDS FOR YOUTH <br /> This AGRE N is made and entered into as of the _��T day of <br /> 201 y and between FRIENDS FOR YOUTH ("ConsultanY'), RE 00 CI <br /> 2020 (a collabora on comprised of eight partners, including City of Redwood, Redwood City <br /> Elementary School District, Sequoia Union High School District, County of San Mateo Heafth and <br /> Human Services, Sequoia Healthcare District, John W. Gardner Center for Youth and their <br /> Communities —Stanford University, Kaiser Permanente, and First 5 of San Mateo County) and CITY <br /> OF REDWOOD ("Cityu) Acting as Fiscal Agent for Redwood Ciry 2020. Collectively Consultant, <br /> Redwood City 2020, and Ciry are hereinafter referred to as the"Parties." <br /> RECITALS <br /> A. Redwood City 2020 is a Community Collaborative that coordinates the delivery of <br /> services to children, youth and families living in the City of Redwood City and North Fair Oaks, an <br /> unincorporated area adjacent to Redwood City. Redwood Ciry 2020 is made up of eight partners, <br /> including City of Redwood City, Redwood City Elementary School District, Sequoia Union High School <br /> District, County of San Mateo Health and Human Services, Sequoia Healthcare District, John W. <br /> Gardner Center for Youth and their Communities—Stanford University, Kaiser Permanente, and First <br /> 5 of San Mateo County to provide leadership and oversight necessary to develop a coordinated <br /> approach to strengthen academic and social outcomes for children and youth living within the <br /> attendance area of the Redwood City School District and provide valuable resources to families so <br /> they can be successful primary caregivers to their children. <br /> B. City serves as fiscal agent for Redwood Ciry 2020 and as such contracts for services <br /> required by Redwood City 2020. <br /> C. Redwood City 2020 requires the professional services of a consultant experienced in <br /> program coordination. <br /> D. Consultant has the necessary experience in providing professional seroices and <br /> advice. <br /> E. Selection of Consultant is expected to achieve the desired results in an expedited <br /> fashion. <br /> F. Consultant has submitted a proposal to Redwood City 2020 and has affirmed its <br /> willingness and ability to perform such work. <br /> NOW, THEREFORE, in consideration of these recitals and the mutual covenants contained <br /> herein,the Parties agree as follows: <br /> 1. Scope of Services ("Services"). <br /> Specific tasks to be completed by Consultant shall include: <br /> • Required Mentor Training for all volunteers about to be matched with a youth-in-need. <br /> Training includes AOD Prevention, abuse prevention, and youth development content as well <br /> as additional material from ongoing Collaborative and Workgroup meetings, helping mentors <br /> better understand the pressures mentees may be facing; <br /> • Specifically, one 2-hour Senior Friend Group offered semi-annualty on how to talk to youth <br /> about tough topics like drugs and afcohol using recently collected AOD data with webinar <br /> follow-up; <br /> REV:10-29-13 MLG <br /> Page 1 of 4 <br /> ATTY/AGR.2013.210/AOD Subcontract with FFY 6 months 12-30-13 <br />
