EXHIBIT "A" <br /> SCOPE OF SERVICES <br /> CONSULTANT will provide, at City's request and direction destruction of finro (2) <br /> groundwater monitoring wells (MW-1, MW-1R) and preparation of site closure report, as <br /> required to comply with the letter from the San Mateo County Groundwater Protection <br /> Program dated January 8, 2014. <br /> 0.1 The performance of all services by CONSULTANT will be to the satisfaction <br /> of the CITY OF REDWOOD CITY. <br /> 0.2 All of the services to be furnished by the CONSULTANT under this <br /> AGREEMENT from the inception of the AGREEMENT until termination of the <br /> AGREEMENT will meet the professional standards and quality which prevail <br /> among project and construction management consultants of similar <br /> knowledge and skill engaged in similar work throughout California under the <br /> same or similar circumstances. All services performed by CONSULTANT will <br /> be in compliance with all applicable federal, state and local codes, rules and <br /> regulations which are in force at the time such services are rendered. <br /> 0.3 CONSULTANT will coordinate all services with and communications between <br /> the REDWOOD CITY's designated Project Manager, CITY's staff, including <br /> engineering and legal staff to ensure compliance with any city adopted <br /> standards ordinances that may affect the outcome of this effort. <br /> 0.4 CONSULTANT will schedule and attend all meetings necessary, and as <br /> directed by REDWOOD CITY's designated Project Manager, in order to <br /> complete all services to the satisfaction of THE CITY OF REDWOOD CITY. <br /> 1 A Services and Deliverables <br /> • Prepare permit application for well destruction and submit to SMCoGPP with <br /> permit fee. <br /> • Check condition of wells, measure water levels and total well depth prior to well <br /> destruction. <br /> • Destroy monitoring welfs by pressure grouting according to SMCoGPP <br /> guidelines. Well boxes will be removed, and asphalt pavement will be restored. <br /> • Prepare a report documenting the work completed. <br /> • Complete DWR forms for welt destruction and submit to CA Department of Water <br /> Resources. <br /> • Upload report to Geotracker and provide electronic copy to clients. <br /> Schedule <br /> Well destruction to be completed by February 28, 2014, with report submitted to <br /> SMCoGPP by March 14, 2014. <br /> 2.0 Comqensation <br /> CONSULTANT shall provide the aforementioned services shall be a lump sum fee of <br /> $5,400. <br /> REV:02-05-14 VR <br /> Page 6 of 6 <br />
