1 <br /> NOW, THEREFORE, in consideration of these recitals and the mutual <br /> oovenants contained herein, the Parties agree as foilows: <br /> ARTICLE 1—BACKGROUND AND OBJECTIVES <br /> Notre Dame seeks funding from Redwood City 2020 to provide one Notre Dame <br /> Mission Volunteers-AmeriCorps member at Sequoia High School who will provide <br /> services in the Sequoia Teen Resource Center for the 2013-2014 school year. <br /> 1. Term. Unless earlier terminated, the term of this MOU is consistent with <br /> 2013-2014 school year, ending on June 30, 2014. <br /> 2. Scope of Services ("Services°). <br /> Notre Dame Mission Volunteers-AmeriCorps will provide one member to provide <br /> services at Sequoia High School's Sequoia Teen Resource Center for the 2013- <br /> 2014 school year. The member shall carry out the following responsibilities: <br /> a) Work with about fifty (50) referred youth during the 2013-14 school year, under <br /> the supervision of the Sequoia Teen Resource Center Director and in <br /> collaboration with the Sequoia High School administration. <br /> b) Complete a baseline profile for each student entering the program. <br /> c) Develop a written action plan to address the speci�c issues that interfere with <br /> each student's school success. Every plan will contain measurable goals such <br /> as consistent attendance at classes, completion of assignments, improvement <br /> in grades/behavior, etc. <br /> d) Meet at least weekly with each assigned student to revisit the goals of the <br /> action plan and ensure focus on goal attainment. <br /> e) Provide school staff members who refer the students with weekly updates (e.g., <br /> the student's participation, efforts, issues, and progress). <br /> f) Help students establish contact with onsite and offsite student support senrices <br /> that promote school connectedness and success. <br /> g) Follow up with referrals for support services to ensure that assigned students <br /> have made connections and are continuing satisfactorily. <br /> h) Facilitate delivery of services through scheduting, helping with permission slips, <br /> and addressing other apparent barriers as appropriate. <br /> i) Maintain complete, confidential, and ongoing communication with appropriate <br /> school staff (teachers, counselors, administrators) as well as assigned <br /> students. <br /> j) Maintain complete records of steps taken to assist assigned students. <br /> REV:03-06-14 MlG <br /> Page 2 of 5 <br /> AlTY/AGR.2014.026/Americorps 13 14 <br />
