Scope of Services that shali be completed by StarVista: <br /> StarVista will work in collaboration with the AOD Abuse Prevention Partnership to build <br /> program capacity in youth serving and mentoring, and to increase the number of youth <br /> involved in positive activities and supportive youthladult relationships. <br /> Specific tasks to be completed include: <br /> • StarVista staff will collaborate with the Sequoia Youth Advisory Board (YAB) to <br /> update the assessment tool with validated AOD survey questions; <br /> • StarVista will coordinate with school staff to allow YAB members to go into <br /> classrooms and administer the Needs Assessment Survey with students. Staff <br /> and youth will present the outcomes of this survey to the Prevention <br /> Partnership and administrative staff at Sequoia High School; <br /> • StarVista staff will be trained to implement the Leadership and Resiliency <br /> program, will recruit at least 15 high-risk youth at Sequoia High School to <br /> participate in this program; <br /> • StarVista will facilitate a Leadership and Resiliency program in the 2014/15 <br /> school year at Sequoia High School, implementing the following 4 program <br /> components: <br /> 1) weekly resiliency group meetings to be held after school, including <br /> expressive art approaches, psycho education on drugs and alcohol, as well <br /> as individual skill building; <br /> 2) community service activities; <br /> 3) peer education; <br /> 4) outdoor activities; <br /> • StarVista staff will attend monthly Prevention Partnership Steering Committee <br /> meetings; and <br /> • StarVista will ensure appropriate County reporting and data collection <br /> requirements are met including but not limited to entering and maintaining <br /> monthly CaIOMS records. <br /> 2. Standard of Performance. While performing the Services, Consultant will <br /> exercise the reasonable professional care and skill customarily exercised by reputable <br /> members of Consultant's profession practicing in the urban Northern Califomia Area, <br /> and will use reasonable diligence and best judgment while exercising its professional <br /> skill and expertise. <br /> 3. Term. Unless earlier terminated, the term of this Agreement will be effective from <br /> date first above written until June 30, 2015. <br /> 4. Schedule.
