The grant is for the explicit purposes outlined in <br /> your grant proposal and is subject to the conditions listed below. To acknowledge this <br /> agreement and accept the grant,please have an officer of your organization read and sign this <br /> agreement and return it to us at Sequoia Healthcare District,525 Veterans Blvd.,Redwood City, <br /> CA 94063 by June 6,2014. <br /> Grant Number: 2014-15-Redwood City 20/20-SRS <br /> Grant Amount. $10,000 <br /> Grant Period: July 1,2014-June 30,2015 <br /> Grant Purpose: RWC 20/20 Safe Routes Coordinator <br /> Payment Schedule: First payment June 20,2014--$5,000 <br /> Second payment February 1,2015--$5,000 <br /> Reporting Schedule: 1) First reporfi due December 18,2014 <br /> 2) Final report due May 2,2015 <br /> Special Conditions: Sequoia Healthcare District Representative may request a site visit <br /> during the grant cycle. <br /> I. Expenditure of Funds <br /> The grantee shall use this grant solely for the purposes stated above,and the program may be <br /> modified only with the written approval of the Sequoia Healthcare District's Board of Directors. <br /> The grantee shall return to the District any unexpended funds: <br /> 1) at the end of the grant period,or <br /> 2) if the Sequoia Healthcare District determines that the grantee has not performed in <br /> accordance with this agreement and the approved program and budget,or <br /> 3) if the grantee loses exempt"Public Charity" status under Section 501 (c) (3) of the <br /> Internal Revenue Code. <br /> The grantee shall maintain adequate supporting records consistent with generally accepted <br /> accounting practices to show the use of these funds. The grantee shall make these books and <br /> records available to the Sequoia Healthcare District in keeping with the grant reporting <br /> schedule described below. <br /> The grantee shall not use any portion of the funds granted herein to influence the <br /> outcome of any specific public election. This grant is not earmarked for lobbying <br /> purposes and no portion of it may be used for lobbying purposes. <br />
