� ORIGINAL <br /> AGREEMENT <br /> CENTER FOR INDEPENDENCE OF INDIVIDUALS WITH DISABILITIES <br /> (Housing Accessibility Modifications) <br /> THIS AGREEMENT, made and entered into this�day of <br /> 2015, by and befinreen the CITY OF REDWOOD CITY, a charter city and municipal <br /> corporation of the State of California ("City") and CENTER FOR INDEPENDENCE <br /> OF INDIVIDUALS WITH DISABILITIES, a California nonprofit organization <br /> ("Contractor"; collectively the "Parties"). <br /> WITNESSETH: <br /> WHEREAS, Contractor provides certain services through programs, <br /> including the program described in Exhibits "A" ("Program & Quantitative Goals"), <br /> and "B" ("Program Budget"), attached hereto and made a part hereof; and <br /> WHEREAS, Contractor has requested financial assistance in order to <br /> conduct the program described in Exhibit A for residents of City during the fiscal <br /> year commencing July 1, 2015, and ending June 30, 2016 (the "Fiscal Year"); and <br /> NOW, THEREFORE, the Parties hereby agree as follows: <br /> 1. FINANCIAL ASSISTANCE. City will allocate to Contractor the sum of <br /> Eighteen Thousand Five Hundred and No/100 Dollars ($18,500) for exclusive use <br /> by Contractor during the fiscal year commencing July 1, 2015, and ending June <br /> 30, 2016, saiely for the purposes described in Paragraph 2. <br /> 2. USE OF FUNDS. Contractor will use the funds provided pursuant to <br /> Paragraph 1 solely for the Project objectives described in Exhibit A and Exhibit B, <br /> attached hereto and by this reference incorporated herein. <br /> 3. CHANGES TO PROJECT.
