ORIGINAL <br /> AGREEMENT <br /> CENTER FOR INDEPENDENCE OF INDIVIDUALS WITH DISABILITIES <br /> (Housing Accessibility Modifications) <br /> THIS AGREEMENT, made and entered into this CC( day of Oda/Li <br /> 2016, by and between the CITY OF REDWOOD CITY, a charter city and municipal <br /> corporation of the State of California ("City") and CENTER FOR INDEPENDENCE <br /> OF INDIVIDUALS WITH DISABILITIES , a California nonprofit organization <br /> ("Contractor" ; collectively the "Parties"). <br /> WITNESSETH : <br /> WHEREAS, Contractor provides certain services through programs, <br /> including the program described in Exhibits "A" ("Program & Quantitative Goals") , <br /> and "B" ("Program Budget") , attached hereto and made a part hereof; and <br /> WHEREAS, Contractor has requested financial assistance in order to <br /> conduct the program described in Exhibit A for residents of City during the fiscal <br /> year commencing July 1 , 2016 , and ending June 30, 2017 (the "Fiscal Year"); and <br /> NOW, THEREFORE, the Parties hereby agree as follows : <br /> 1 . FINANCIAL ASSISTANCE . City will allocate to Contractor the sum of <br /> Fifteen Thousand and No/100 Dollars ($ 15,000) for exclusive use by Contractor <br /> during the fiscal year commencing July 1 , 2016 , and ending June 30, 2017 , solely <br /> for the purposes described in Paragraph 2 . <br /> 2. USE OF FUNDS . Contractor will use the funds provided pursuant to <br /> Paragraph 1 solely for the Project objectives described in Exhibit A and Exhibit B, <br /> attached hereto and by this reference incorporated herein . <br /> 3. CHANGES TO PROJECT.
