<br /> ORIGINAL <br /> AGREEMENT <br /> [FAMILY SERVICE AGENCY/SENIOR CASE MANAGEMENT AND ADVOCACY] <br /> THIS AGREEMENT, made and entered into this J ro1Y1 day of August, 2004, by and <br />between the CITY OF REDWOOD CITY, a charter city and municipal corporation of the State of <br />California ("City") and FAMILY SERVICE AGENCY OF SAN MATEO COUNTY/SENIOR CASE <br />MANAGEMENT AND ADVOCACY, a California nonprofit organization ("Contractor"); <br /> WIT N E SSE T H: <br /> WHEREAS, Contractor provides bilingual Spanish-speaking case management to help seniors <br />connect with community resources, both public and private, so that they can maintain their independence <br />and well-being in the community; and <br /> WHEREAS, Contractor has requested financial assistance in order to conduct the program <br />for residents of City during the fiscal year commencing July 1, 2004, and ending June 30, 2005; <br />and <br /> WHEREAS, City has reviewed said request and desires to allocate to Contractor the sum <br />of Seven Thousand Eight Hundred and No/100 Dollars ($7,800); <br /> NOW, THEREFORE, the parties hereto do hereby agree as follows: <br /> 1. FINANCIAL ASSISTANCE. City shall allocate to Contractor the sum of Seven <br />Thousand Eight Hundred and No/100 Dollars ($7,800) for exclusive use by Contractor during the <br />fiscal year commencing July 1, 2004, and ending June 30, 2005, solely for the purposes <br />described in paragraph 2. <br /> 2. USE OF FUNDS. Contractor shall use the funds provided pursuant to paragraph 1 <br />solely for the Program purposes described in Exhibit "A" (Contract Objectives) attached hereto <br />and by this reference incorporated herein. <br /> 3. CHANGES TO PROGRAM.
