() ~ <br />serVi~iS made and entered into as of the ~ day of ~ r, <br /> 5 c <br />199~, etween the CITY OF REDWOOD CITY, a municipal corporation <br />of the State of California (" City ") and OCCUPATIONAL HEALTH <br />SERVICES ("OHS ") to amend the Agreement For Employee Assistance <br />Program Services between City and OHS entered into on July 11, <br />1994. <br /> 1. City and OHS agree that Section 1 of Addendum Two, <br />Consideration Payable by Employer, is deleted in its entirety and <br />the following is inserted in its place: <br /> "l. Monthly Prepayment Fees. Prepayment fees payable by <br />Employer to OHS are calculated on a capitated basis based on the <br />number of Members enrolled as of the first day of each month <br />during the term of this Group Agreement as follows: <br /> $2.10 per Subscriber per month. II <br /> 2. City and OHS agree that Section 3 of Addendum Three, <br />Benefits and Services, is hereby deleted in its entirety and the <br />following is inserted in its place: <br /> II 3. Benefits and services include: <br /> a) Six (6 ) counseling sessions per family unit <br /> b) Management referral/consultation up to a maximum <br /> of six (6 ) sessions <br /> r <br />
