<br />DES:d-ik 02/18/93 <br /> <br />(003/32) <br /> <br />0 RIG I N A L <br /> <br />3/8/93 <br /> <br />RES 0 L UTI 0 N <br /> <br />N O. <br /> <br />11948 <br /> <br />---------- <br /> <br />RESOLUTION APPROVING AND AUTHORIZING EXECUTION <br />AGREEMENT FOR EMPLOYEE ASSISTANCE PROGRAM SERVICES <br /> <br />OF <br /> <br />(OCCUPATIONAL HEALTH SERVICES, INC.) <br /> <br />WHEREAS, <br /> <br />this <br /> <br />Council <br /> <br />desires <br /> <br />to <br /> <br />provide <br /> <br />a <br /> <br />program <br /> <br />of <br /> <br />counseling services to employees of the City of Redwood City with <br /> <br />various personal, medical-behavioral problems, in order that said <br /> <br />employees more effectively may perform their employment; and <br /> <br />WHEREAS, Occupational Health Services, Inc., represents that <br /> <br />it is qualified to provide such expert professional services; and <br /> <br />WHEREAS, <br /> <br />Occupational <br /> <br />Health <br /> <br />Services, <br /> <br />Inc. <br /> <br />has <br /> <br />been <br /> <br />providing the services set forth in the attached agreement since <br /> <br />1985; and <br /> <br />WHEREAS, there has been presented to, and reviewed by, this <br /> <br />Council an agreement by and between the City of Redwood City and <br /> <br />Occupational <br /> <br />Health <br /> <br />Services, <br /> <br />Inc. , <br /> <br />in <br /> <br />furtherance <br /> <br />of <br /> <br />the <br /> <br />aforesaid purposes; <br /> <br />NOW THEREFORE: <br /> <br />BE IT RESOLVED BY THE COUNCIL OF THE CITY OF REDWOOD CITY, <br /> <br />AS FOLLOWS: <br /> <br />That certain agreement dated as of January 1, <br /> <br />1993, by and <br /> <br />between the City of Redwood City, a municipal corporation of the <br /> <br />State of California, <br /> <br />and Occupational Health Services, <br /> <br />Inc. , <br /> <br />a <br /> <br />California <br /> <br />corporation <br /> <br />entitled <br /> <br />"Agreement <br /> <br />for <br /> <br />Employee <br />
