ORIGINAL TASK ORDER/ADDITIONAL SERVICES NO, 4 This Task Order is entered into pursuant to the terms and conditions of the following described agreement (the "Agreement"), entered into by and between the City of Redwood City, a municipal corporation of the State of California (the "City") and the contractor (the "Contractor") hereinafter identified: . Title of Agreement: Professional Consulting Services Contractor: Subsurface Consultants, Inc, 3736 Mt. Diablo Blvd" Suite 200 Lafayette, CA 94549-3659 Date of Agreement: Sept. 12, 1996 1. Task Described, Contractor shall perform the following services, which are in addition to those services described in the Agreement. Description of Services: Environmental Site Characterization and report preparation, Phase One 2. Compensation. City shall compensate Contractor for the services described in this Task Order under one of the two following alternatives, as indicated (check and complete, as appropriate): a) ~ (check, if appropriate) Compensation shall be based upon the rates of charge and method of calculation described in the Agreement. (See Contractor's Table 1 - Fee Estimate, schedule of hourly rates.) Compensation shall be provided as follows: Dollar value of Task Order No.4 not to exceed $80,340.00. 3. Time of Performance. Contractor shall perform the services to be provided pursuant to this Task Order on or before the following: Date of Completion: Extend date of completion from 8/01/2000 to June 30, 2001. 4. Approval.
