<br />ORIGINAL <br /> <br />AGREEMENT <br /> <br />Center for Independence of Disabled <br />Housing Accessibility Modification Program <br /> <br />THIS AGREEMENT, made and entered into this Eday of ~2005, by <br />and between the CITY OF REDWOOD CITY, a charter city and municipal corporation of the <br />State of California ("City") and Center for Independence of Disabled, a California nonprofit <br /> <br />organization ("Contractor") (collectively the "Parties"); <br /> <br />WITNESSETH: <br /> <br />WHEREAS, Contractor provides housing accessibility modifications to individuals <br />with disabilities who are residents of the City; and <br /> <br />WHEREAS, Contractor has requested financial assistance in order to complete the <br />Project that will benefit residents of City during the fiscal year commencing July 1, 2005, and <br /> <br />ending June 30, 2006; and <br />WHEREAS, City has reviewed said request and the Project and desires to allocate to <br /> <br />Contractor the sum of Thirty Thousand and no/100 Dollars ($30,000) from its Community <br />Development Block Grant funds for the Project; <br /> <br />NOW, THEREFORE, the Parties hereby agree as follows: <br /> <br />1. <br /> <br />FINANCIAL ASSISTANCE. City will allocate to Contractor the sum of Thirty <br /> <br />Thousand and no/100 Dollars ($30,000) for exclusive use by Contractor during the fiscal <br />year commencing July 1, 2005, and ending June 30, 2006, solely for the purposes <br /> <br />described in Paragraph 2. <br /> <br />2. <br /> <br />USE OF FUNDS. Contractor will use the funds provided pursuant to Paragraph <br /> <br />1 solely for the Project purposes described in Exhibits A (Budget) and Exhibit B (Contractor <br /> <br />Objectives) and Exhibit C (Environmental Reviews for Housing Accessibility Modification <br /> <br />(HAM) Projects for Center for Independence of the Disabled, attached hereto and by this <br />reference incorporated herein. <br /> <br />CDBG/DJT.cje <br /> <br />7/19/2005 <br />
