<br />DES:dft n8/20/90 (010/29) <br /> <br />ODlf:'i[\1 I'd <br />i \ i \.:U ~ td~ti- <br /> <br />AUG 2 7 1990 <br /> <br />RES 0 L UTI 0 N <br /> <br />No.11386 <br /> <br />RESOLUTION APPROVING AND AUTHORIZING EXECUTION OF <br />AGREEMENT FOR EMPLOYEE ASSISTANCE PROGRAM SERVICES <br /> <br />(OCCUPATIONAL HEALTH SERVICES, INC.) <br /> <br />WHEREAS, this Council desires to provide a program of <br /> <br />counseling services to employees of the City of Redwood City with <br /> <br />various personal, medica I-behavioral problems, in order that said <br /> <br />employees more effectively may perform their employment; <br /> <br />WHEREAS, Occupational Health Services, Inc., represents that <br /> <br />it is qualified to provide such expert professional services; <br /> <br />WHEREAS, there has been presented to, and reviewed by, this <br /> <br />Council an agreement entitled "Agreement for Employee Assistance <br /> <br />Program Services", by and between the City of Redwood City and <br /> <br />Occupational Health Services, Inc., in furtherance of the <br /> <br />aforesaid purposes; <br /> <br />NOW THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE CITY OF <br /> <br />REDWOOD CITY AS FOLLOWS: <br /> <br />That certain agreement dated as of January 1, 1990, by and <br /> <br />between the City of Redwood City, a municipal corporation of the <br /> <br />State of California, and Occupational Health Services, Inc., a <br /> <br />California <br /> <br />corporation <br /> <br />entitled <br /> <br />"Agreement <br /> <br />for <br /> <br />Employee <br /> <br />Assistance Program Services" (form dated 08/20/90) providing <br /> <br />personal counseling services for employees of the City of Redwood <br /> <br />City, a copy of which agreement is on file in the office of the <br /> <br />City Clerk, to which copy reference is hereby made for the full <br />
