The goals (Exhibit A) and budget (Exhibit B) are attached. In order to <br />assure compliance with applicable provisions of the Internal Revenue Code, we must obtain the <br />Grantee's written agreement to each of the following terms and conditions of this grant: <br />(1) All of the grant funds are to be used for, and only for, the purpose of supporting the <br />SPIN/Shinnyo Fellow. Use of any portion of the grant funds, including any interest eamed <br />thereon, for any other purpose must have the prior written approval of the Foundation. <br />(2) The grant funds must be used only in accordance with the budget submitted as part of your <br />grant request, a copy of which is attached to this letter as Exhibit B. Any changes in the <br />budget or any modifications to other grant conditions must be authorized by prior written <br />approval of the Foundation. <br />(3) Narrative report(s) and corresponding financial statement(s) must be furnished on the <br />date(s) indicated herein on the progress made in accomplishing the purpose of the grant. <br />Financial statements must describe the expenditures of the grant funds in accordance with <br />the submitted budget.
