4/6/98 <br /> <br /> ORIGINAL <br /> <br /> RESOLUTION NO. 13305 <br /> <br /> RESOLUTION APPROVING AND AUTHORIZING EXECUTION <br /> OF AGREEMENT FOR OCCUPATIONAL HEALTH AND <br /> SAFETY SERVICES <br /> <br /> (PERMANENTE MEDICAL GROUP, INC.) <br /> <br />BE IT RESOLVED BY THE COUNCIL OF THE CITY OF REDWOOD <br />CITY, AS FOLLOWS: <br /> <br /> That certain agreement entitled, "Letter of Agreement For Occupational <br />Health and Safety Services Between the Permanente Medical Group, Inc. and <br />City of Redwood City", by and between the City of Redwood City, a municipal <br />corporation of the State of California, and Kaiser Permanente Occupational <br />Health Services, a division of Kaiser Foundation Health Plan, Inc., a nonprofit, <br />public benefit California corporation, for the performance of occupational health <br />and safety services, a copy of which letter agreement is on file in the office of the <br />City Clerk, to which copy reference is hereby made for the full particulars thereof, <br />is hereby approved, and the City Manager and City Clerk are hereby authorized <br />and directed to execute, and to attest thereto, respectively, said agreement for <br />and on behalf of the City of Redwood City. <br /> <br /> Reso-375 <br /> FXSS:djk 13305 <br /> ~Shared/Redwood/Council <br /> 03~23~98 <br /> <br /> <br />
