onfginci AMENDMENT TO AGREEMENT BETWEEN THE COUNTY OF SAN MATEO AND CITY OF REDWOOD CITY THIS AMENDMENT TO THE AGREEMENT, entered into this � day ofl , , 2019, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and City of Redwood City, hereinafter called "Contractor"; WITNESSETH: WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof; WHEREAS, on December 20, 2016 the parties entered into an Agreement, Resolution 074891h, for Contractor to provide Core Agency Services to at risk and homeless residents for the term of November 15, 2016 to June 30, 2019 for a total obligation of $595,105; and WHEREAS, the parties wish to amend the Agreement to extend existing services and the tern of the agreement through June 30, 2021 and add $463,896 for an amount not to exceed, $1,059,001. NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS: 1. Paragraph 1. Exhibits and Attachments of the Agreement is hereby amended as follows: Exhibit A - Services Exhibit B - Payments and Rates (revised June 2019) Exhibit C - Performance Reporting (revised June 2019) Exhibit D — Clarity Human Services System Usage and Data Sharing Agreement for Core Service Agencies and Homeless Service Providers Attachment I - § 504 Compliance Attachment P — Personally Identifiable Information 2. Paragraph 3. Payments of the Agreement is hereby amended to read as follows: In consideration of the services provided by Contractor in accordance with all terms, conditions, and specifications set forth in this agreement and in Exhibit A, County shall make payment to Contractor based on the rates and in the manner specified in Exhibit B (revised June 2019).
