<br />4 -Day Vote Center Use Agreement Form
<br />Tuesday, June 7th, 2022 — Statewide Direct Primary Election
<br />-Facility Name Fair Oaks Community Center - 2600 Middlefield Road, Redwood City,
<br />CA 94063
<br />The facility described above ("Facility") agrees to host a Vote Center on June 7, 2022 ("Election Day")
<br />and for three days prior to Election Day.
<br />The Vote Center at the Facility will remain open to the public for no less than eight (8) hours per day,
<br />including weekends and holidays. The schedule of hours for the voting public shall be from 9 a.m. to
<br />5 p.m. on June 4, 2022 through June 6, 2022 and from 7 a.m. to 8 p.m. on June 7, 2022. The Facility
<br />shall be accessible to persons with disabilities as defined in the Americans with Disabilities Act of
<br />1990, as amended. The Vote Center location shall not be changed without written authorization from
<br />a Supervisor from the San Mateo County Registration & Elections Division ("Division"), the Assistant
<br />Chief Elections Officer, or the Chief Elections Officer ("Authorized Staff').
<br />Access to the Facility will be granted to Vote Center personnel from 8:00 a.m. to 6:00 p.m. from June
<br />4, 2022 to June 6, 2022. On June 7, 2022, Vote Center workers will be granted access to the Facility
<br />from 6:00 a.m. to 11:00 p.m.
<br />Vote Center workers, or other employees of the Division, will be granted access for the set-up of
<br />equipment and removal of equipment from Facility. Please indicate your preference below:
<br />Four hours on Thursday, June 2, 2022 and/or Friday, June 3, 2022 for the set-up of equipment,
<br />and four hours on Wednesday, June 8, 2022 and/or Thursday, June 9, 2022 for the removal of
<br />equipment.
<br />❑Four hours on Friday, June 3, 2022 for the set-up of equipment and four hours on Wednesday,
<br />June 8, 2022 for the removal of equipment.
<br />The Facility will be contacted with a finalized delivery window two (2) weeks prior to the delivery date.
