GRANT AGREEMENT <br />FOR <br />CALIFORNIA ACCOUNTABLE COMMUNITIES FOR HEALTH INITIATIVE <br />Grantee: City of Redwood City (acting as the fiscal agent for Redwood City Together) <br /> Grant #: 35650968 <br />Grant Amount: $400,000 <br />Grant Period: July 1, 2023 – December 31, 2025 <br />1.Grant Purpose: Community Partners entered into an agreement with the California Department of Public <br />Health (“CDPH”) on November 16, 2022 (the “CDPH Agreement”), to partner with established Accountable <br />Communities for Health (ACHs) and implement a competitive grant process to award grants for the <br />establishment of up to 25 more ACHs in new parts of the State. The California Accountable Communities for <br />Health Initiative (“CACHI”) grant funding will support activities to accelerate progress toward the goals and <br />milestones of an ACH, including building cross-sector partnerships to develop shared outcomes; establishing <br />governance and distributing leadership; elevating the role of community leaders; identifying and <br />implementing strategies to achieve greater health equity; and developing strategies to blend and braid <br />resources. In addition, grantees will identify a state initiative as the area of focus for collaboration, which will <br />enable the ACH to become engaged with an effort that holds potential for providing additional and ongoing <br />financial support to the ACH. <br />Grantee agrees to: <br />a.Make progress along the developmental pathway of an ACH through implementation of ACH goals and <br />milestones (Appendix 1). <br />b.Collaborate and work on one state initiative at the local level as an area of focus where the ACH can play <br />an important role and show value to the success of the initiative. <br />c.Engage actively in the CACHI learning community by attending virtual and in person meetings, contributing <br />materials and documents, sharing lessons learned, and offering peer exchange to new ACHs and the <br />broader field. <br />d.Participate in coaching, check-in meetings and technical assistance. <br />e.Participate in the initiative-wide evaluation, including annual site visits and regular phone calls, as well as <br />completion of surveys and milestone reports. <br />f.Provide a commitment of matching resources (in-kind or funding) totaling a minimum of $100,000 over <br />the grant period.
