REV: 11-27-24 LF <br />MEMBERSHIP AGREEMENT <br />KAISER FOUNDATION HEALTH PLAN, INC. <br />REDWOOD CITY TOGETHER, AND THE CITY OF REDWOOD CITY SERVING AS <br />FISCAL AGENT FOR <br />REDWOOD CITY TOGETHER <br />FISCAL YEARS 2024-2026 <br />This is a Membership Agreement (“Agreement”), made and entered in this ____ <br />day of _____________, 20___, by and between Kaiser Foundation Health Plan, Inc. <br />(“Kaiser Permanente”), Redwood City Together (Redwood City Together), a community <br />collaborative that coordinates the delivery of services to children, youth and families living <br />in the City of Redwood City and North Fair Oaks, an unincorporated area adjacent to City <br />of Redwood City, and THE CITY OF REDWOOD CITY, a charter city and municipal <br />corporation of the State of California (“City”) serving as Fiscal Agent for Redwood City <br />Together (collectively, “the Parties”). <br />WITNESSETH: <br />WHEREAS, in 1995 the City, the Redwood City School District, the Sequoia Union <br />High School District and the County of San Mateo formed Redwood City 2020 (then <br />known as Redwood City 2000) to provide leadership and oversight necessary to develop <br />a coordinated approach to strengthen academic and social outcomes for children and <br />youth living within the attendance area of the Redwood City School District and provide <br />valuable resources to families so they can be successful primary caregivers to their <br />children; and <br />WHEREAS, each year since the formation of Redwood City 2020, these original <br />collaborative partners (the City, Redwood City School District, Sequoia Union High <br />School District and County of San Mateo) have each annually renewed their membership <br />in Redwood City 2020 by formal agreement; and <br />WHEREAS, over time, the original collaborative partners who formed Redwood <br />City 2020 have been joined by Cañada College, John W. Gardner Center for Youth and <br />Their Communities at Stanford University (“Stanford University”), Kaiser Permanente, <br />and Sequoia Healthcare District as additional collaborative partners; and <br />WHEREAS, since joining, these additional collaborative partners (Cañada <br />College, John W. Gardner Center for Youth and Their Communities at Stanford University <br />(Stanford University), Kaiser Permanente, and Sequoia Healthcare District) have each <br />annually renewed their membership in Redwood City 2020 by formal agreement; and <br />WHEREAS, in January 2021, the Redwood City 2020 Leadership Council <br />members voted to change Redwood City 2020’s name to “Redwood City Together”; and <br />WHEREAS, during Fiscal years 2021-2022, Chan Zuckerberg Initiative and North <br />Fair Oaks Community Council joined and have since opted not to renew their membership <br />in Redwood City Together; and <br />ATTY/AGR.2024.222/Kaiser Permanente (Membership Agreement FY24-26) (Page 1 of 11) <br />10th <br />January 25
