REV: 02-06-26 LF <br />Memorandum of Agreement <br />Between <br />California Accountable Communities for Health Initiative (CACHI), <br />Redwood City Together <br />And <br />City of Redwood City (acting as fiscal agent for Redwood City Together) <br />This Memorandum of Agreement outlines the shared goals, commitments, activities, <br />responsibilities, and expectations of the partnership between the California Accountable <br />Communities for Health Initiative (“CACHI”), Redwood City Together and the City of <br />Redwood City acting as fiscal agent for Redwood City Together. The partnership between <br />CACHI, Redwood City Together and the City of Redwood City acting as fiscal agent for Redwood <br />City Together is a mutually beneficial partnership designed to support the ACH (defined below) <br />and foster CACHI’s strategic priorities. <br />1)Parties: This Memorandum of Agreement (“MOA”) is an agreement between CACHI, <br />Redwood City Together and City of Redwood City acting as fiscal agent for Redwood City <br />Together. <br />2)Purpose: The purpose of this MOA is to establish a clear understanding of shared goals, <br />commitments, activities, responsibilities, and expectations of the partnership between <br />CACHI and Redwood City Together. This MOA also establishes the scope of work <br />assumed by both parties during the duration of the MOA. <br />3)Term of MOA: The term of this MOA is from January 1, 2026, to December 31, 2026, <br />unless terminated earlier by either party as specified in Section 10 below. <br />4)Points of Contact: CACHI and Redwood City Together will each designate a Point of <br />Contact (POC) to facilitate implementation of this MOA. The POCs are authorized to act <br />on behalf of their respective organizations for activities that are outlined in this MOA. The <br />Current POCs are listed below: <br />i) Rea Pañares, Executive Director, CACHI <br />ii) Rafael Avendaño, Executive Director, Redwood City Together <br />5)Shared goals and commitments: <br />a)Shared goals: The goals of the partnership between CACHI and Redwood City <br />Together are to: <br />i)Strengthen, support, and sustain the ACH’s development and practice as <br />an ACH to advance its mission, vision, and local objectives <br />ii)Track, monitor, and evaluate the progress and accomplishments of the <br />ACH <br />iii)Foster the activation and mobilization of the ACH Network comprising the <br />ACHs across California around key issues, events, and opportunities <br />ATTY/AGR.2026.033/California Accountable Communities for Health Initiative (ACH 2026) (Page 1 of 4)
