ORIGINAL <br /> AGREEMENT <br /> YOUTH AND FAMILY ENRICHMENT SERVICES (YFES) — CRISIS INTERVENTION AND SUICIDE <br /> PREVENTION CENTER <br /> THIS AGREEMENT, made and entered into this day of 010, by and <br /> between the CITY OF REDWOOD CITY, a charter city and a municipal corporation of the State of <br /> California ( "City") and YOUTH AND FAMILY ENRICHMENT SERVICES (YFES) — CRISIS <br /> INTERVENTION AND SUICIDE PREVENTION CENTER, a California nonprofit organization <br /> ( "Contractor "); <br /> WITNESSETH: <br /> WHEREAS, Contractor provides multiple hotlines including a 24 -hour suicide and crisis hotline, a <br /> 24 -hour drug and alcohol hotline, and a parent support hotline; and on -call counselors who are able to <br /> respond to crisis situations in schools; and <br /> WHEREAS, Contractor has requested financial assistance in order to conduct the program for <br /> residents of City during the fiscal year commencing July 1, 2010, and ending June 30, 2011; and <br /> WHEREAS, City has reviewed said request and desires to allocate to Contractor the sum of Eight <br /> Thousand and No /100 Dollars ($8,000.00); <br /> NOW, THEREFORE, the parties hereto do hereby agree as follows: <br /> 1. FINANCIAL ASSISTANCE. City shall allocate to Contractor the sum of Eight Thousand <br /> No /100 Dollars ($8,000.00) for exclusive use by Contractor during the fiscal year commencing July 1, <br /> 2010, and ending June 30, 2011, solely for the purposes described in paragraph 2. <br /> 2. USE OF FUNDS. Contractor shall use the funds provided pursuant to paragraph 1 solely <br /> for the Program purposes described in Exhibit "A" (Contract Objectives) attached hereto and by this <br /> reference incorporated herein. <br /> 3. CHANGES TO PROGRAM. No changes in the Program described in this agreement which <br /> are funded by the financial assistance provided under paragraph 1 shall be made without the prior written <br /> consent of City. <br />
