ORIGINAL <br /> <br /> AGREEMENT <br /> <br /> CREATE A SMILE FOUNDATION <br /> <br /> THIS AGREEMENT, made and entered into this t'~ day of ~'Cua~, 2001, by and <br />between the CITY OF REDWOOD CITY, a municipal corporation of the State of California <br />and CREATE A SMILE FOUNDATION, a California nonprofit organization; <br /> <br /> WITNESSETH: <br /> <br /> WHEREAS, Contractor provides emergency assistance to Redwood City residents who <br />become victims of tragedy, violence, etc.; and <br /> WHEREAS, Contractor has requested financial assistance in order to conduct <br />emergency services ("Program") for residents of Redwood City during the fiscal year <br />commencing July 1, 2000, and ending June 30, 2001; and <br /> <br /> WHEREAS, City has reviewed said request and desires to allocate to Contractor the sum <br /> of Five Thousand and No/100 Dollars ($5,000); <br /> <br /> NOW, THEREFORE, the parties hereto agree as follows: <br /> 1. FINANCIAL ASSISTANCE. City shall allocate to Contractor the sum of Five <br /> Thousand and No/100 Dollars ($5,000) for exclusive use by Contractor during the fiscal year <br /> commencing July 1, 2000, and ending June 30, 2001, solely for the purposes described in <br /> paragraph 2. <br /> 2. USE OF FUNDS. Contractor shall use the funds provided pursuant to paragraph 1 <br /> solely for the Program purposes described in Exhibit "A" (Contract Objectives) attached hereto <br /> and by this reference incorporated herein. <br /> 3. CHANGES TO PROGRAM.
