� �RE�OiVAL FUNDING AGREEMENT FOR HUMAN SERVICES FINANCIAL ASSISTANCE PROGRAM BETWEEN THE CITY OF REDWOOD CITY AND SAMARITAN HOUSE THIS AGREEMENT, made and entered into this � day of ��✓�,/2011, by and belween the CITY OF REDWOOD CITY, a charter city and a muniapal corporat�on of the State of California ("City") and SAMARITAN HOUSE, a California nonprofit organization ("Contractor'), WITNESSETH WHEREAS, Contractor provides free healthcare services to low-income individuals and famiiies who do not receive medical insurances through their low-wage �obs, cannot afford "out of pocket" heafthcare expenses, and have no other access to healthcare services, and WHEREAS, Contractor has requested financial assistance in order to conduct the program for residents of City dunng the fiscal year commenc�ng July 1, 2011, and ending June 30, 2012, and WHEREAS, City has reviewed said request and desires to allocate to Contractor the sum of Seventeen Thousand and No/100 Dollars ($17,000 00) NOW, THEREFORE, the parties hereto do hereby agree as foliows 1 FINANCIAL ASSISTANCE City shall allocate to Contractor the sum of Seventeen Thousand and No/100 Dollars ($17,000 00) for exclusive use by Contractor during the fiscal year commencing July 1, 2011, and ending June 30, 2012, solely for the purposes described in paragraph 2 2 USE OF FUNDS Contractor shall use the funds provided pursuant to paragraph 1 solely for the Program purposes descnbed in Exhibit "A" (Contract Ob�ectives) attached hereto and by this referenceincorporated herem 3 CHANGES TO PROGRAM No changes in the Program described in this agreement which are funded by the financial assistance prowded under paragraph 1 shall be made without the prior written consent of City 4 PAYMENTS Contractor shail keep detailed and accurate records of all expenditures made and expenses incurred which are funded under this agreement Payments of funds allocated
