ORIGINAL <br /> <br /> AGREEMENT <br /> <br /> Ombudsman Services of San Mateo County <br /> <br /> Ombudsman Services <br /> <br /> THIS AGREEMENT, made and entered into this day of C. AJCE.U¢--~. <br /> <br />2003, by and between the CITY OF REDWOOD CITY, a charter city and municipal <br />corporation of the State of CalifOrnia ("City") and Ombudsman Services of San Mateo <br />County, a California nonprofit organization ("Contractor'') (collectively the "Parties"). <br />WlTNESSETH: <br /> <br /> WHEREAS, Contractor provides advocacy services to the residents of long-term <br />care facilities; and <br /> <br /> WHEREAS, Contractor has requested financial assistance in order to conduct <br />the program described in the attached Exhibit B (Contract Objectives),. which is <br />incorporated herein by this reference (the "Program"), for residents of City during the <br />fiscal year commencing July 1,2003, and ending June 30, 2004; and <br /> <br /> WHEREAS, City Council has reviewed Contractor's request; finds that providing <br />the financial assistance will benefit Redwood City and its citizens; and, desires to <br /> <br />allocate to Contractor the sum of Seven Thousand and no/100 Dollars ($7,000). <br /> NOW, THEREFORE, the Parties hereby agree as follows: <br /> <br /> 1. FINANCIAL ASSISTANCE. City will allocate to Contractor the sum of <br />Seven Thousand and no/100 Dollars ($7,000) for exclusive use by Contractor during the <br />fiscal year commencing July 1,2003, and ending June 30, 2004, solely for the purposes <br />described in Paragraph 2. <br /> <br /> 2. USE OF FUNDS. Contractor will use the funds provided pursuant to <br />Paragraph 1 solely for the Program purposes described in Exhibits A (Budget) & B. <br /> <br />9/11/03 <br /> <br /> <br />
