Parks, Recreation and 1400 Roosevelt Avenue <br /> Community Services Redwood City, California 94061 <br /> Department Mr� Phone (650) 780-7250 <br /> �� FAX (650) 368-5087 <br /> www.redwoodcity.org <br /> August 13, 2012 <br /> Leslie Crane <br /> North Star Academy <br /> 4Q0 Duane Street <br /> Redwood City, CA 94062 <br /> Dear Ms. Crane, <br /> The Redwood City Civic Cultural Commission hereby enters into an agreement with <br /> North Star Academy to provide funding for one performing arts production to be held <br /> during the 2012-2013 season. This agreement is subject to the following terms and <br /> conditions and shall become effective upon acceptance thereof by North Star <br /> Academy (hereinafter called "Contractor"). <br /> 1. SERVICES OF CONTRACTOR. <br /> Contractor shall: <br /> a. Receive and coordinate expenditure of funds as determined for said <br /> organtzation; <br /> b. Manage and coordinate all entsrtainment, vendors, permits, sound systems, and <br /> other logistics considerations for the said events; <br /> c. Coordinate all rental and use arrangements with the management of the facilities <br /> which the organization is using; <br /> d. Supply any equipment needed by the organization, vendors or entertainers; <br /> e. List the Redwood City Civic Cultural Commfssion as a co-sponsor of the event. <br /> The Civic Cultural Commissfon MUST be listed on all publicity pieces, flyers, and <br /> press releases. The Civic Cultural Commission must receive copies of all <br /> publicity materials released by North Star Academy regarding this event; <br /> f. Provide passes for alt seven Cultural Commissioners to attend the funded event. <br /> If the event is offered at no charge, contractor wiil provide invitation or flyer to <br /> Commissioners to remind them of event location, time and date; <br /> g. After the programs have taken place, the Contractor must submit a written <br /> report regarding the attendance at each event and response to the program, as <br /> well as a financial statement showing how the grant funds were spent; <br /> h. Use g�anted funds for the purpose of funding one performing arts production <br /> during the 2012-2013 school year. Money not used for this purpose by June 30, <br /> 2013 must be returned to the Civic Cultural Commission. <br />
