ORIGINAL <br /> AGREEMENT <br /> OMBUDSMAN SERVICES OF SAN MATEO COUNTY, INC <br /> (Ombudsman Services) <br /> THIS AGREEMENT, made and entered into this 1Cday of /," <br /> 2016, by and between the CITY OF REDWOOD CITY, a charter city and municipal <br /> corporation of the State of California ("City") and OMBUDSMAN SERVICES OF <br /> SAN MATEO COUNTY, INC, a California nonprofit organization ("Contractor"; <br /> collectively the "Parties") . <br /> WITNESSETH : <br /> WHEREAS, Contractor provides certain services through programs, <br /> including the program described in Exhibits "A" ("Program & Quantitative Goals") , <br /> and "B" ("Program Budget") , attached hereto and made a part hereof; and <br /> WHEREAS, Contractor has requested financial assistance in order to <br /> conduct the program described in Exhibit A for residents of City during the fiscal <br /> year commencing July 1 , 2016, and ending June 30, 2017 (the "Fiscal Year") ; and <br /> NOW, THEREFORE, the Parties hereby agree as follows: <br /> 1 . FINANCIAL ASSISTANCE. City will allocate to Contractor the sum of <br /> Fifteen Thousand Dollars ($ 15, 000. 00) for exclusive use by Contractor during the <br /> fiscal year commencing July 1 , 2016, and ending June 30, 2017, solely for the <br /> purposes described in Paragraph 2 . <br /> 2 . USE OF FUNDS. Contractor will use the funds provided pursuant to <br /> Paragraph 1 solely for the Project objectives described in Exhibit A and Exhibit B , <br /> attached hereto and by this reference incorporated herein . <br /> 3 . CHANGES TO PROJECT.
