<br /> ~"'ISION SER"\7IC'E PL~' <br /> CALIFORNIA VISION SERVICE <br /> A Non-Profit Corporation <br /> doing business as <br /> VISION SERVICE PLAN <br /> (Hereinafter Called VSP) <br /> and <br /> CITY OF REDWOOD CITY <br /> (Hereinafter Called Group) <br />AGREE AS FOLLOWS: <br />VSP shall provide a panel of doctors to perform services to the covered <br />persons, as described under "Eligibility Determination", subject to the <br />terms and conditions of this contract. <br />TE~~ AND CANCELLATION <br />This contract shall be effective commencing November 1 , 1987 for an <br />Initial Term of twenty-four (24) months, and continuing thereafter <br />until terminated by either party giving the other sixty (60) days' prior <br />written notice. <br />The contract shall terminate at midnight, the last day of the month for <br />which the notice has been given. <br />AGREEMENT <br />This instrument contains a~l of the provisions of the agreement between the <br />parties hereto, and no promise or·agreement not contained herein shall be <br />binding on the parties unless the same is in writing, signed by the partiës <br />hereto and attached to this contract. This contract may be amended only by <br />a written instrument signed by an authorized representative of Group and an <br />officer or director of VSP. Consent of covered person is not required to <br />effect any such amendment. <br />The provisions and conditions set forth on the following seventeen (17) <br />pages shall form a part of this contract as fully as if recited over the <br />signatures hereto affixed. <br />IN WITNESS w~REOF, the parties have caused this contract to be executed at <br />Sacramento, California. <br />CITY OF REDWOOD CITY CALIFORNIA VISION SERVICE <br /> .
