ORIGINAL <br />AGREEMENT <br />CENTER FOR INDEPENDENCE OF INDIVIDUALS WITH DISABILITIES <br />Housing Accessibility Modifications (HAM) <br />THIS AGREEMENT, made and entered into this NO day of November <br />2017, by and between the CITY OF REDWOOD CITY, a charter city and municipal <br />corporation of the State of California ("City") and CENTER FOR INDEPENDENCE <br />OF INDIVIDUALS WITH DISABILITIES <br />("Contractor"; collectively the "Parties'). <br />a California nonprofit organization <br />WITNESSETH: <br />WHEREAS, Contractor provides certain services through programs, <br />including the program described in Exhibits "A" ('Program & Quantitative Goals"), <br />and `B" ('Program Budget'), attached hereto and made a part hereof; and <br />WHEREAS, Contractor has requested financial assistance in order to <br />conduct the program described in Exhibit A for residents of City during the fiscal <br />year commencing July 1, 2017, and ending June 30, 2018 (the "Fiscal Year"); and <br />NOW, THEREFORE, the Parties hereby agree as follows: <br />1. FINANCIAL ASSISTANCE. City will allocate to Contractor the sum of <br />Fifteen Thousand and No/100 Dollars ($15,000) for exclusive use by Contractor <br />during the fiscal year commencing July 1, 2017, and ending June 30, 2018, solely <br />for the purposes described in Paragraph 2. <br />2. USE OF FUNDS. Contractor will use the funds provided pursuant to <br />Paragraph 1 solely for the Project objectives described in Exhibit A and Exhibit B, <br />attached hereto and by this reference incorporated herein. <br />3. CHANGES TO PROJECT.
