DES:djk ./16/96 (003/40) <br /> 0 R I G I N A L 1/29/96 <br /> <br /> RESOLUTION NO. 12687 <br /> <br /> RESOLUTION APPROVING AND AUTHORIZING EXECUTION OF <br /> AMENDMENT TO AGREEMENT FOR EMPLOYEE ASSISTANCE PROGRAM <br /> SERVICES <br /> <br /> (OCCUPATIONAL HEALTH SERVICES, INC.) <br /> <br /> WHEREAS, the City of Redwood City and Occupational Health <br />Services, Inc., entered into an agreement dated July 11, 1994 <br />(the "Basic Agreement") pursuant to which Occupational Health <br />Services, Inc. has provided a program of counseling services to <br />employees of the City of Redwood City with various personal, <br />medical-behavioral problems, in order that said employees more <br />effectively may perform their employment; and <br /> <br /> WHEREAS, pursuant to paragraph VI.E., of the Basic Agreement <br />the parties renewed and extended the term and revised the <br />compensation thereunder pursuant to Amendment thereto dated <br />January 23, 1995; and <br /> <br /> WHEREAS, the parties to the Basic Agreement desire to renew <br />and extend the term thereof and to revise the compensation <br />payable thereunder for the period January 1, 1996 through <br />December 31, 1996; and <br /> <br /> WHEREAS, a Second Amendment to the Agreement for Employee <br />Assistance Program Services has been presented to, and reviewed <br />by, this Council; <br /> <br /> NOW, THEREFORE: <br /> <br /> BE IT RESOLVED BY THE COUNCIL OF THE CITY OF REDWOOD CITY, <br />AS FOLLOWS: <br /> <br /> That certain agreement by and between the City of Redwood <br />City, a municipal corporation of the State of California, and <br /> <br /> <br />
