Curbside Voting Service workers, or other employees of the <br />Registration & Elections Division ("Division"), will arrive onsite on Friday, October 30, 2020 to setup <br />and remain onsite. Equipment will be retrieved by Wednesday, November 4, 2020. <br />Payment for the use of Facility shall be as indicated below: <br />X Facility agrees to waive payment. <br />❑ Full payment required. $ per day that the Curbside Voting Service is open to the <br />public. <br />(Facility must provide W-9) <br />❑ Partial payment. $ per day that the Curbside Voting Service is open to the public. <br />(Facility must provide W-9) <br />Can your facility provide three (3) tables for our use? <br />Yes X No ❑ <br />Can your facility provide six (6) chairs for our use?
