REV: 10-26-2021 SK Exhibit “A” to MLA Form of Site License Authorization This Site License Authorization (“SLA”) is approved by the city of Redwood City (“City”) this _______ day of _________, 20___ (the date executed by all parties, referred herein as “SLA Effective Date”). This SLA, dated as of the date set forth below, is entered into by and between the City and Crown Castle Fiber, a Limited Liability Company, (“Licensee”) and is pursuant to that certain Municipal Facilities Master License Agreement dated ________, 20__ (the “MLA”) - MLA Contract ID 2488400. 1. SLA. Licensee has submitted a request for an encroachment permit in conjunction with this application for an SLA pursuant to the MLA. City has reviewed the application to use Municipal Facility and grants approval subject to the terms of this SLA. All of the terms and conditions of the MLA are incorporated hereby by reference and made a part hereof without the necessity of repeating or attaching the MLA. In the event of a contradiction, modification, or inconsistency between the terms of the MLA and this SLA, the terms of this SLA shall govern. Capitalized terms used in this SLA shall have the same meaning described for them in the MLA unless otherwise indicated herein. 2.Site Name/No.:______________ 3. Site Address: ____________________ 4. This SLA is not a building permit or other regulatory approval to install any Equipment (as defined in the MLA). 5. Equipment and Locations. Licensee shall have the right to attach the Equipment described in Attachment 1 to the specific space on the specific Municipal Facility identified in Attachment 2. 6. Term. The term of this SLA shall be as set forth in Section 2 of the MLA. 7. Annual Fees. Licensee shall pay City an Annual Fee in accordance with Section 4.1 and Exhibit B of the MLA. 8.Installation Date. The earlier of ninety (90) days from full execution of an SLA, or the date that any Equipment is first installed by Licensee pursuant to an SLA issued by the City in accordance with the MLA. If the latter, Licensee shall document in writing the date by notice to the City in accordance with Section 1.4 of the MLA. 9. Termination. Licensee upon tendering one hundred and eighty (180) days written notice to the other party may terminate this SLA. City may terminate this SLA upon a default by the other Party of any material covenant or term of this SLA which default is not cured within forty-five (45) days of receipt of written notice of default (or, if such default is not curable within forty-five (45) days, if the defaulting Party fails to commence that cure within forty-five (45) days or fails thereafter diligently to prosecute such cure to completion); provided, that the grace period for any ATTY/AGR.2021.268/Crown Castle (Page 25 of 28) SF REDWOOD CITY 043M2 871 WILLOW ST (ON BAY RD) November 24 21 8th April 26
