AGREEMENT O/~/(~//~ <br /> Catholic Charities CYO of the Archdiocese of San Francisco <br /> <br /> San Carlos Adult Day Support Center <br /> <br /> THIS AGREEMENT, made and entered into this'-~day of (~)~ , <br />2003, by and between the CITY OF REDWOOD CITY, a charter city and municipal <br />corporation of the State of California ("City") and Catholic Charities CYO of the <br />Archdiocese of San Francisco, a California nonprofit organization ("Contractor") <br />(collectively the "Parties"). <br />WITNESSETH: <br /> <br /> WHEREAS, Contractor provides adult day support services; and <br /> <br /> WHEREAS, Contractor has requested financial assistance in order to conduct <br />the program described in the attached Exhibit B (Contract Objectives), which is <br />incorporated herein by this reference (the "Program"), for residents of City during the <br />fiscal year commencing July 1,2003, and ending June 30, 2004; and <br /> <br /> WHEREAS, City Council has reviewed Contractor's request; finds that providing <br />the financial assistance will benefit Redwood City and its citizens; and, desires to <br /> <br />allocate to Contractor the sum of Five Thousand no/100 Dollars ($5,000). <br /> NOW, THEREFORE, the Parties hereby agree as follows: <br /> <br /> 1. FINANCIAL ASSISTANCE. City will allocate to Contractor the sum of Five <br />Thousand no/100 Dollars ($5,000) for exclusive use by Contractor during the fiscal year <br />commencing July 1, 2003, and ending June 30, 2004, solely for the purposes described <br />in Paragraph 2. <br /> <br /> 2. USE OF FUNDS. Contractor will use the funds provided pursuant to <br />Paragraph 1 solely for the Program purposes described in Exhibits A (Budget) & B. <br /> <br />9/11/03 <br /> <br /> <br />
