EXHIBIT"A" SCOPE OF SERVICES Project Description The purpose af these design enginee�ing services is to prepare construction documents for joint trench undergrounding composifie drawings and the Joint Trench Form B for the Middlefield Road Underground Utility District (MUUD 2011-01). The (imits of work are as shown an the MUUD 2011-01 Boundary Description appraved by City CounciL The composite drawing's canduits and utility boxes/vaults wilf be based on the MUUD loint 7rench Intent Drawings,in conjunction with tt�e designs provided by the util'ity companies. Astaund, PG&E, Gomcast and RT&T. In addition,the project will include separate plans far the undergrounding a�f the electrical services to the existing street lights on the existing power poles. Services TASK 1—SUPPLEMENTA!FIEtQ SURVEYING Consultant wi[I perfarm field surveying to supplement the aerial tapography for the MUUD project flown in November 2011. The field surveying is to tie-i� more precise locations at the project conforms and at visible facilities or obstructions along the joint trench alignment. The surveying will be performed by a union journeyman fi�ld crew in two separate move-ins, using the same project survey control that was set for the ae�ial topagraphy, where passible. Field survey data will be processed by Consultant office surveyors under the directian of a licensed Iand surveyor and downloaded into AutoCAD 2Q13 for use by the engineers. Task 1a—Gravitv Inverts and Topa First Move-in:The first surveying move-in will be during the . 65% design period after the initial ob�tructions are identified. The items to be surveyed are facilities determined by the engineer to be in close proximity to the joint trench and/ar that have an impact on the joint trench plan and profile. Examples of surveyed facilities are water valves, power poles, (ight poles, drainage inlets, manholes, fire hydrants,fence lines, trees and ather items identified during the 65% design.Also key for the first survey move-in will be the surveying of the gravity sewer and storm drain line inverts. The inverts will infarm the designers for the profiling of the new joint trench vs the existing gravity lines. The inverts af the existing storm drain and sanitary will be surveyed at lacations in proximity to a joint trench crossing. This task wiil also include recard mapping updates on the side streets,to be requested from the City. Task 1b—Gravitv Inverts and Topo Second Move-in: A second move-in for field surveying wili be performed at the end of the 95%design to collect location of facilities obsenred in follow-up site walks and identified during 95% design period. The secand moye-in may also require follow-up for gravity lines inve�ts. TASK 2—FIElD SURVEYING OF PG&E SERVKF LATERALS Consultant wifl perform field surveying of the PG&E lateral markers at 120 private property service trenches. The services trenches are new conduits and termination cans to be installed by PG&E in Spring 2015, PG&E will be stubbing the joint trench conduits into a box or vault at the edge of the praperty, and this box will be the connection point between the MUUD project and the PG&E private praperty trenches. Consultant surveyors will tie-in the newly installed boxes, ff the private property trenches are not installed at the time of the City bidding of the MUUD project,then Consultant will use a portion of this budget to review the PG&E service trench plans and approximate the location of the service lateral stubs and vaults.
