___ � . <br /> ORIGII�4� T� <br /> AMENDMENT NO. 1 TO AGREEMENT FOR PROFESSIONAL SERVICES TO <br /> CONDUCT THE 2015 CITY SATISFACTION SURVEY <br /> (Godbe Research) <br /> T ' m ndment No 1 is entered into and effective as of the ��day of <br /> , 2015, amending the agreement dated April 30, 2b15 (the <br /> "Agreem t") and between the City of Redwood City, a charter city and municipal <br /> corporatio (" ity"), and Godbe Research, ("Consultant") (collectively, the "Parties") to <br /> conduct the 15 city satisfaction survey. <br /> RECITALS <br /> A. On April 30, 2015, the Parties executed Agreement for Professional Services <br /> to Conduct the 2015 City Satisfaction Survey; and <br /> B. The Parties have negotiated and agreed to a supplemental fee schedule, <br /> which is attached to and incorporated by this reference as Exhibit "A," Fee. <br /> NOW, THEREFORE, in consideration of these recitals and the mutual covenants <br /> contained herein, the Parties agree as follows: <br /> 1. In addition to those services contained in the Agreement, as may have <br /> been amended from time to time, Consultant will provide those services described in <br /> Exhibit "A." <br /> 2. City will pay Consultant for all work associated with those services <br /> described in Exhibit "A" as stated in original agreement, finrelve hundred dollars <br /> ($1,200), for an aggregate amount of ($30,775) for the completion of all the work and <br /> services described herein, for the completion of 22-minute telephone survey (previously <br /> 20-minute telephone survey as stated in original agreement). <br /> 3. Consultant will adhere to the schedule set forth in Exhibit "A" as stated in � <br /> original agreement. <br /> 4. All other provisions of the Agreement, as may have been amended from <br /> time to time, will remain in full force and effect. <br /> 5. All requisite insurance policies to be maintained by Consultant pursuant to <br /> the Agreement, as may have been amended from time to time, will include coverage for <br /> this Amendment. <br /> 6.
