Call the Facilities Help Line at(650) 780-7473 during business hours, or for after <br /> hour emergencies call Communications at (650) 780-7118 and request that they <br /> notify a Facilities Maintenance staff person. <br /> H. Schedules and Records Required <br /> 1. Contractors shall submit preventative maintenance work schedules to the City. These <br /> work schedules shall be designed for each type of equipment to be serviced, such as <br /> traction elevators or hydraulic elevators, and shall conform to the manufacturer's <br /> recommended practice for the particular equipment concerned. They shall show the kind <br /> and frequency of service and lubrication proposed by the Contractor for a one year <br /> duration. The City will evaluate and approve the proposed maintenance schedules before <br /> the work can commence. <br /> 2. All inspections, lubrication, adjustments, tests, cleaning, routine repairs, and other <br /> prevenrive maintenance activities shall be performed in accordance with the approved <br /> maintenance schedules. <br /> 3. The Contractor shall keep these work schedules on display in each equipment room. <br /> Schedules posted shall be of the chart type, which shall be initialed by the service person <br /> when each scheduled inspection is performed. <br /> 4. Contractor agrees to provide an Activity Log in a permanently bound journal having pre- <br /> numbered pages and to keep the log current. Log is to be kept in the machine room and <br /> available for review by Owner at all times. All entries to be in ink, be legible, <br /> consecutive, and without blanks.Activity Log is the property of Owner. The Activity Log <br /> will be complimentary to the Maintenance &Lubrication Logs. Entries will identify who <br /> from the contractor's organization was in the building, during what period of time, and <br /> for what puipose (i.e. regular maintenance, regular time call-back, overtime call-back, <br /> etc.). Equipment failures, passenger entrapments or unusual maintenance situations are to <br /> be noted and require the insertion of a brief explanatory entry. <br /> 5. A service/repair tag is to be filled out by the Contractor for each billable repair that is not <br /> part of the contract. A copy of this shall be given to the City's maintenance mechanic if <br /> present or it should be faxed to Facilities Management by the next business day. <br /> I. Hours of Service <br /> All normal work under this agreement will be performed during regular hours of regular working <br /> days of the elevator trade. (Regular work days and hours are Monday through Friday 7:30 a.m. <br /> to 4:30 p.m.) If overtime work is required, and is authorized by Owner, Owner will pay <br /> contractor only the difference between normal and overtime labor at Contractor's regular billing <br /> rate.
