officer at any time and at such frequency or intervals as is <br /> aesirable, a statement giving the name and address of the owner or <br /> tenant of �ach and every one of the premises where a chemical toilet <br /> shall have been. cleaned out by said registrant or his employees or <br /> by others on his behalf and said statement sh.a11 also describe in <br /> precise terms the place �nere the clear_ings shall have been disposed <br /> of and by whom. The �epartment may require such statements to be <br /> sworn to before a notary. <br /> SECTSON ll. A change of business or residence address of <br /> any registrant, including a member of a partnership which is reg- <br /> istered, shall be reported in writing by registered mail by the <br /> registrant �rithin two (2� days after said change of address. <br /> S�CTION 12. Any registration issued under this section may <br /> be revoked by the Department for cause on ten (10) days notice to ap- <br /> plicant, which notice shall be served by regiatered mail or in person <br /> at the latest place of residence or of business reported by the ap- <br /> plicant. <br /> SECTZON 13. The San Mateo County Departinent of Public <br /> Heal.th and Welfare is authorized to issue rules and regulations in <br /> order to administer and carry out the purposes of this ordinance. <br /> SECTIGN 1I�. It shall be unlawful to violate any of the pro- <br /> visions of this ordinance or any or�.er or orders the Department made <br /> pursuant to this ordinance for the protection of human health and comfort. <br /> SECTION 15. Any person violating any oi the previsions of <br /> this ordinance shall be deemed guilty of a misdemeanor �.nd upon <br /> conviction thereof, shall be punished by a fine not exceeding Five <br /> Hundred Dollars (��500.00) , or by imprisonment in the County Jail of <br /> the County of San ljTateo, State of California, not exceeding ninety <br /> (90) days, or by both such. fine and imprisonment. <br /> S�CTION 16.
