The term of the original agreement
is from July 1, 2013 through June 30, 2016.

The proposed amendment constitutes the third amendment to the original contract.

ALTERNATIVES
Council could choose not to approve the amendment, and not accept the additional
funding from the San Mateo County Human Services Agency for the Fair Oaks
Community Center Information and Referral Program.

FISCAL IMPACT
The proposed amendment provides a total of $84,780 for FY 2014-2015 and an
additional amount of up to $16,718 of Sustainability Funds for the term of the agreement
at the discretion of the Director of the Human Services Agency. These amounts have
been submitted as mid-year budget adjustments for FY 2015-2016.

ENVIRONMENTAL REVIEW
This activity is not a project under CEQA as defined in CEQA Guidelines, section
15378, because it has no potential resulting in either a direct or foreseeable physical
change in the environment.

TERI CHIN,
HUMAN SERVICES MANAGER

CHRIS BETH
PARKS AND RECREATION SERVICES DIRECTOR
6.1.A. - Page 2
