PrQgz�ms must maintain a minimum eighty-five perce�t attendance rate for eaeh of the funding strearns. If program attendance cirops be�ow 85°r'o during two calendar years, (four attenc�ance periods), the grant will be recalculated and reduced. If program attendance draps below ?5°lo at any of the attendance periods, the grant will be reduced accordingly. Dua� funded si#es need ta achieve 100°lo attendance in ASE� and 85-1QQ% in 21'` GCLC. The state Af�er School EducatiQn and Safety grants program requires program attendance to be reporteci twice a year — January 31 and 3uly 31. When praviders report attendance to the Redwood City School Dist�ict, they must use the CDE grant attendance forms. Providers will also submit a copy of a �ityspan report verify�ng the numbers they submit and sign this copy of the report agreeing to the reported nurnbers. In arder to have time to review and submit the forms to th� state, the forms must be completed by the provider az�d return�d to the After Sc�aaol Coordinator two weeks in adr�ance of the state due date. ATTENDANCE FORMS ARE DUE TO THE DTSTRIGT: Janu�ry IS—for period July 1 --December 31 July 1S—for period January 1 —June 30 � Attendance (totai student� served} is c�iculated by ad�iing tageth�r the number of stud�nts who participated each day af the pxogram ciuring the reporting period. For exarr�ple, if 3Q stuc�ents participated Manday, 4Q Tuesday, 35 Wednesday, 40 Thursday and 20 Friday, then the total students served that week is 165. The form also asks for the number of days the program was in aperation durir�g the progra�n period. Far middle schoal programs, althaugh the state anly requires students to attend 3 days per week, your attendance rate is based on a 5-day a w�ek pragram. Thus if students anly attend 3 clays a week, Contractor must�Il the other two days with students to achieve ma.xitr�um attenda�ce. Each pravid�r must keep accurate and detailed attendance records that suppc�rt the attendance reported �o the District, t1�e st�te and federal govemment. These records sha11 be made available to the I)istrict at the District's requesf. It is the Contractor's resp+ansibility to make sure al1 attend�nce information is suhmitted accurately and is identical to the Cityspan system.
