8.1.E. - Page 9 <br /> EXHIBIT A — SCOPE OF SERVICES <br /> 1. General Guidelines and Procedures <br /> 1.1. Licensing <br /> Contractor shall be licensed by the appropriate State and/or local regulatory agency. <br /> The Contractor shall secure and pay for all permits, fees, and licenses necessary for <br /> the proper execution and completion of the work. The Contractor shall observe and <br /> comply with all federal, state, county, and city laws, codes, ordinances, rules and <br /> regulations in accomplishing the work. <br /> The General Contractor shall be in possession of a valid, active California Class "A" <br /> (General Engineering) Contractor's License. <br /> 1.2. Work Order and Response Times <br /> Contractor shall only perform utility trench repair work upon receipt of a written <br /> work order from the Public Works Director or his/her designee. Utility trench repair <br /> work shall be completed as soon as possible and in all cases within 21 calendar days <br /> or less of Contractor receiving notification, unless extended by the City in writing. <br /> 1.3. Staffing <br /> Contractor shall be responsible for providing the appropriate types and skill levels of <br /> personnel, which is considered customary in the industry, required to accomplish the <br /> work. <br /> Contractor shall be represented by a competent foreman or superintendent. The <br /> Contractor may not change his superintendent without obtaining prior written <br /> approval from the City. The Contractor is requested to identify in writing the <br /> authority of their onsite superintendent, and identify in writing the person(s) who will <br /> have the full capability of acting on behalf of the Contractor. <br /> The Contractor shall designate, in writing, the names and telephone numbers of at <br /> least three representatives who could be contacted at any time in the event that an <br /> emergency occurs. <br /> 1.4. Parts and Materials <br /> Contractor must possess, and have readily available in functioning order, all required <br /> tools, equipment, apparatus, facilities, and materials needed to perform all work <br /> necessary to repair the City's utility trenches. Parts or materials to be incorporated in <br /> the work shall be stored in such a manner as to ensure the preservation of their quality <br /> and fitness for the work. <br /> All material shall be disposed of at the Contractors expense.
