Attachment A <br />The mid -year report should contain the following: <br />1. Progress on your fundraising efforts to -date detailing all donations of $5,000 or more <br />2. Progress on groundbreaking and construction activities <br />3. A copy of all news releases and press stories that you have generated on the project <br />especially where SHD is mentioned <br />4. Your overall assessment on how well you feel that you are progressing towards meeting your <br />goals and objectives. If you are behind schedule indicate why and how you plan to move <br />forward to meet all goals and objectives. <br />5.
