Fax -!10 _ a SCOPE OF SERVICES Contractor will perform rodent and cockroach control services for the City for the course of the Agreement. I. Acronyms A. City — City of Redwood City B. SMCMVCD —San Mateo County Mosquito and Vector Control District H. General Provisions A. SMCMVCD Insuection Fee: Contractor will pay an annual inspection fee of two dollars per bait station to SMCMVCD. The inspection fee is based upon the number of bait stations and the frequency in which they are to be serviced. The Contractor shall recover this fee only through the Inspection of Bait Station fee and not as a separate charge. B. License: Contractor will possess a Qualified Applicator License, Category A, (Residential, Industrial and Institutional), or Category K (Health Related), issued by the State of California, Department of Pesticide Regulation, Pesticide Enforcement Branch, or a Branch "2" Pest Control Operator's License issued by the Structural Pest Control Board of the State of California, in force during the performance of all work under this Agreement. Contractor will inform the City if the license expires, is revoked or otherwise becomes invalid. C. Safetv and Health Reeulations: Contractor will comply with all provisions of the United States Department of Labor, Occupational Safety and Health Administration, Part 1910, Occupational Safety and Health Standards when work is to be performed in confined space areas. Full compensation for securing the required permit(s) when working in confined space areas shall be considered as included in the prices paid for the various contract items and no additional compensation will be paid. D. Review and Anoroval of Work: All work shall be performed to the satisfaction of San Mateo County Mosquito and Vector Control District, 1351 Rollins Rd, Burlingame, CA 94010, (650) 344-8592. If Contractor's work is rejected, Contractor must re -inspect all work at no cost to the City. All deficiencies must be corrected within five (5) days of notification. E. Records of Work: Contractor must maintain a written record (made in the field at time of work performed) of the items listed below. Records must be submitted to SMCMVCD on the original field maps along with the Contractor's work summary sheet within two (2) working days after completion of each phase of work, prior to billing.
