EXHIBIT A Scope of Work/ Tasks Contractor will furnish all labor, materials, tools, equipment and incidentals to perform concrete sidewalk offset cutting. This work includes: 1. Mobilization and demobilization at project sites; 2. Surveying project sites; 3. Traffic control (as needed); 4. Concrete cutting of sidewalk offsets measuring over 3/4" and below 2"; 5. Reporting locations of all sidewalk offsets needing removal and replacement to the City, 6. Reporting details of all sidewalk offsets addressed, including location, address, date, description, height of offset, length of offset, and calculated cost; 7. Site clean-up. Invoicine Contractor will submit monthly invoices to: Public Works Services Department, Attn: Public Works Superintendent - ROW, 1400 Broadway Street, Redwood City, CA 94063. California State Contractor's License Contractor shall maintain for the duration of the Agreement and any and all amendments, an active contractor's license from the California Contractor's State License Board related to concrete services. 2. Payment Terms The total not to exceed contract amount is $50,000. Fees will be billed based on the actual per inch foot of sidewalk offset removed at each work site at the rate of $18.00 per inch foot of sidewalk offset. Method of Measurement and Payment Inch -feet shall be calculated by multiplying the average depth of the cut on each sidewalk offset by the length along the joint that is cut.
