Enclosed is <br />the proposal form. The contract is to provide for any and all labor, travel time and expenses, <br />and equipment necessary to perform the work+. Failure to use the format provided will result in <br />the proposal being deemed incomplete. <br />Proposals should be completed and delivered in a sealed envelope to the City of Redwood <br />City, Public Works Services Department, located at 1400 Broadway, Redwood City, CA <br />94063 clearly marked "REQUEST FOR PROPOSAL: UTILITY SYSTEMS PUMP REPAIR <br />SERVICES" no later than 3:00 p.m. on Wednesday, September 15,2004. (No facsimile <br />or electronicallv submitted proposals will be accepted.) <br />The City can extend this contract for one (1) consecutive year at the same terms, conditions, <br />and prices. The extension may be exercised by the City Manager in writing. The extension <br />will be based upon satisfactory review of the contractor's performance, the City's needs, and <br />the appropriation of funds by the City Council. A sample of the Agreement is included as an <br />attachment to this RFP. <br />The City of Redwood City reserves the right to reject or accept any proposal or any part or <br />portion there of or any combination of services, and further reserves the right to waive any <br />informality or honest clerical error in any proposal. <br />If you need additional information pertaining to this proposal, please call Rich Boyer, <br />Supervisor, Storm Drains Maintenance at (650) 780-7473 or John Fochetti, Acting Supervisor, <br />Water Utility System at (650) 780-7469. <br />Sincerely, <br />( ~ <br />1h~ß/~ <br />Marilyn W.
