<br />~ <br />ORIGINAL <br />THIRD AMENDMENT TO AGREEMENT <br />FOR EMPLOYEE ASSISTANCE PROGRAM SERVICES <br />THIS AMENDMENT TO AGREEMENT, made and entered into this \ ~\'" <br />day of ~. 1997, by and between the CITY OF REDWOOD CITY, a <br />municipal corporation of the State of California, ("City") and the MANAGED <br />HEALTH NET'NORK (former!y OccupatjDnal Health Services, Inc. "OHS"), a <br />Califomia corporation ("MHT); <br />WITNESSETH~ <br />WHEREAS, the parties hereto have entered into an agreement dated July- <br />11, 1994 (the "Basic Agreement") pursuant to which MHN has provided a <br />program of counseling services to employees of the City of Redwood City with <br />various personal, medical-behavior problems, in order that said employees more <br />effectively may perform their employment; and <br />WHEREAS, pursuant to paragraph VI.E., of the Basic Agreement the <br />parties renewed and extended the term and revised the compensation <br />thereunder pursuant to Amendment thereto dated January 23, 1995; and <br />WHEREAS, pursuant to paragraph VI.E., of the Basic Agreement the <br />parties renewed and extended the term and revised the cGmpensation and the <br />-~, <br />
