<br />Benefits and Services provided under Section 3 of Addendum Three thereunder <br />pursuant to a Second Amendment thereto dated February 23, 1996; and <br /> WHEREAS, the parties hereto desire to renew and extend the term hereof <br />and to revise said Benefits and Services provided under Section 3 of Addendum <br />Three and to add the Department of Corporations Consumer Complaint toll- <br />free number under "Grievance Procedures" of the Combined Evidence of <br />Coverage and Disclosure Form hereunder for the period January 1, 1997 <br />through December 31, 1997; <br />NOW, THEREFORE, the parties hereto agree as follows: <br /> 1. City and MHN agree that Section 3 of Addendum Three, Benefits <br />and Services, is hereby deleted in its entirety and the following is inserted in its <br />place: <br /> "3.
