Additional hours beyond the <br /> twelve (12) shall be billed at $125.00 per hour <br /> e) Quarterly statistical reports". <br />2. The Combined Evidence of Coverage and Disclosure Form under <br />"Grievance Procedures" is hereby amended to read as follows: <br /> "DEPARTMENT OF CORPORATIONS CONSUMER COMPLAINT TOLL FEE NUMBER <br /> If you remain dissatisfied after completing MHTs intemal grievance <br /> and appeals process, you may register your _ complaint with the <br /> California Department of Corporations. <br /> The Califomia Department of Corporations is responsible for <br /> regulating health care service plans. The department has a toll- <br /> free telephone number (1-800-400-0815) to receive complaints <br /> regarding the health plan. If you have a grievance against the <br /> health plan, you should contact the plan and use the plan's <br /> grievance process.
