<br /> VISION SERVICE PLAN <br /> FACILITIES <br /> This vision care plan is an agreement among various state vision service <br /> corporations throughout the United States. Through these non-profit service <br /> plans and their panels of over 14,000 doctors, VSP provides professional <br /> vision care to employees and dependents covered under group vision care <br /> plans. <br /> A list of Panel Doctors in the covered person's geographical location will <br /> accompany the benefit form sent as a result of the covered person completing <br /> the request card attached to the brochure and verification of their <br /> eligibility. This list contains the names, addresses and telephone numbers <br /> of the Panel Doctors.
