<br /> Volunteers: Currently three volunteers are donating 4-8 hours per week to assist clients <br /> in completing forms and doing translations. <br /> Advisory Groups: The Fair Oaks Community Center Advisory Board is made up of <br /> seven (7) members; five of which must reside in the target area, one which represents an <br /> agency housed in the Center and one that represents the community at large. The Board <br /> meets the 2nd Tuesday of each month at 7:00 p.m. in the Community Center. They advise <br /> the Center Coordinator on all phases of the Center's operation. <br /> Board of Directors: City Council of the City of Redwood City. <br />IV. CONTRACTOR RESPONSmILITIES <br /> A. Provide a system through which recipients of service shall have the opportunity to <br /> express and have considered their views, grievances, and complaints regarding the <br /> delivery of services. <br /> B. Keep the San Mateo County Human Services Agency informed about its services and <br /> activities under this contract, and to accept appropriately referred clients from the <br /> San Mateo County Human Services Agency for its contract services as part of its <br /> client base. <br /> C. Contractor agrees to cooperate with the State Department of Social Services (SDSS) <br /> and the San Mateo County Human Services Agency on the implementation, <br /> monitoring, and evaluation of this project and to comply with any and all reporting <br /> and evaluation requirements established by SDSS. <br /> D. Contractor shall establish a procedure to ensure that all employees, volunteers, con- <br /> sultants, subcontractors, or agents perlorming services under this contract report child <br /> abuse or neglect to a child protective agency as defined in Penal Code <br /> Section 11165(k). <br /> E. Contractor shall provide the Human Services Agency with a current budget which <br /> clearly states both the program budget and the total agency budget, if different. In <br /> addition, the contractor will maintain all pertinent reGords and data collection forms <br /> that reflect activities listed in this contract for a period of three (3) years. <br />f0cc94,sn' <br /> -4- <br />
